The Eighth World Congress of Pediatric Cardiology and Cardiac Surgery (WCPCCS) will be held in Washington DC, USA, from Saturday, 26 August, 2023 to Friday, 1 September, 2023, inclusive. The Eighth World Congress of Pediatric Cardiology and Cardiac Surgery will be the largest and most comprehensive scientific meeting dedicated to paediatric and congenital cardiac care ever held. At the time of the writing of this manuscript, The Eighth World Congress of Pediatric Cardiology and Cardiac Surgery has 5,037 registered attendees (and rising) from 117 countries, a truly diverse and international faculty of over 925 individuals from 89 countries, over 2,000 individual abstracts and poster presenters from 101 countries, and a Best Abstract Competition featuring 153 oral abstracts from 34 countries. The purpose of this manuscript is to review the activities related to global health and advocacy that will occur at the Eighth World Congress of Pediatric Cardiology and Cardiac Surgery.
Acknowledging the need for urgent change, we wanted to take the opportunity to bring a common voice to the global community and issue the Washington DC WCPCCS Call to Action on Addressing the Global Burden of Pediatric and Congenital Heart Diseases. A copy of this Washington DC WCPCCS Call to Action is provided in the Appendix of this manuscript. This Washington DC WCPCCS Call to Action is an initiative aimed at increasing awareness of the global burden, promoting the development of sustainable care systems, and improving access to high quality and equitable healthcare for children with heart disease as well as adults with congenital heart disease worldwide.

Vast disparities between and within American states’ responses to the COVID-19 pandemic have evoked renewed attention to whether greater centralization might enhance investments in subnational capacity and remedy subnational inequalities or instead erode subnational organizational capacity. Developments in American public education (1997–2015) offer perspective on this puzzle, which we examine by applying interrupted time series analysis to a novel dataset to assess the implications of centralization on subnational investments in administrative and technical capacity, two dimensions of organizational capacity. We find simultaneous subnational erosion in administrative capacity and growth in technical capacity following centralization, both of which appear concentrated in low-poverty areas despite centralization’s explicit antipoverty purposes. Public education reforms highlight both the challenge of dismantling subnational inequality through centralization and the need for future research on policy designs that enable centralization to yield subnational capacity that is able to remedy inequality.

Background
We have previously shown that the minor alleles of vascular endothelial growth factor A (VEGFA) single-nucleotide polymorphism rs833069 and superoxide dismutase 2 (SOD2) single-nucleotide polymorphism rs2758331 are both associated with improved transplant-free survival after surgery for CHD in infants, but the underlying mechanisms are unknown. We hypothesised that one or both of these minor alleles are associated with better systemic ventricular function, resulting in improved survival.
MethodsThis study is a follow-up analysis of 422 non-syndromic CHD patients who underwent neonatal cardiac surgery with cardiopulmonary bypass. Echocardiographic reports were reviewed. Systemic ventricular function was subjectively categorised as normal, or as mildly, moderately, or severely depressed. The change in function was calculated as the change from the preoperative study to the last available study. Stepwise linear regression, adjusting for covariates, was performed for the outcome of change in ventricular function. Model comparison was performed using Akaike’s information criterion. Only variables that improved the model prediction of change in systemic ventricular function were retained in the final model.
ResultsGenetic and echocardiographic data were available for 335/422 subjects (79%). Of them, 33 (9.9%) developed worse systemic ventricular function during a mean follow-up period of 13.5 years. After covariate adjustment, the presence of the VEGFA minor allele was associated with preserved ventricular function (p=0.011).
ConclusionsThese data support the hypothesis that the mechanism by which the VEGFA single-nucleotide polymorphism rs833069 minor allele improves survival may be the preservation of ventricular function. Further studies are needed to validate this genotype–phenotype association and to determine whether this mechanism is related to increased vascular endothelial growth factor production.

In the United States alone, ∼14,000 children are hospitalised annually with acute heart failure. The science and art of caring for these patients continues to evolve. The International Pediatric Heart Failure Summit of Johns Hopkins All Children’s Heart Institute was held on February 4 and 5, 2015. The 2015 International Pediatric Heart Failure Summit of Johns Hopkins All Children’s Heart Institute was funded through the Andrews/Daicoff Cardiovascular Program Endowment, a philanthropic collaboration between All Children’s Hospital and the Morsani College of Medicine at the University of South Florida (USF). Sponsored by All Children’s Hospital Andrews/Daicoff Cardiovascular Program, the International Pediatric Heart Failure Summit assembled leaders in clinical and scientific disciplines related to paediatric heart failure and created a multi-disciplinary “think-tank”. The purpose of this manuscript is to summarise the lessons from the 2015 International Pediatric Heart Failure Summit of Johns Hopkins All Children’s Heart Institute, to describe the “state of the art” of the treatment of paediatric cardiac failure, and to discuss future directions for research in the domain of paediatric cardiac failure.

Monitoring multidrug-resistant organisms (MDROs) and the infections they cause in a healthcare setting is important to detect newly emerging antimicrobial resistance profiles, to identify vulnerable patient populations, and to assess the need for and effectiveness of interventions; however, it is unclear which metrics are the best, because most of the metrics are not standardized. This document describes useful and practical metrics and surveillance considerations for measuring MDROs and the infections they cause in the practice of infection prevention and control in healthcare settings. These metrics are designed to aid healthcare workers in documenting trends over time within their facility and should not be used for interfacility comparison.
Neuropsychological functioning in adolescent marijuana users: Subtle deficits detectable after a month of abstinence

In adults, studies examining the long-lasting cognitive effects of marijuana use demonstrate subtle deficits in attention, executive function, and memory. Because neuromaturation continues through adolescence, these results cannot necessarily generalize to adolescent marijuana users. The goal of this study was to examine neuropsychological functioning in abstinent marijuana using and demographically similar control adolescents. Data were collected from 65 adolescent marijuana users (n = 31, 26% females) and controls (n = 34, 26% females) 16–18 years of age. Extensive exclusionary criteria included independent psychiatric, medical, and neurologic disorders. Neuropsychological assessments were conducted after > 23 days of monitored abstinence. After controlling for lifetime alcohol use and depressive symptoms, adolescent marijuana users demonstrated slower psychomotor speed (p < .05), and poorer complex attention (p < .04), story memory (p < .04), and planning and sequencing ability (p < .001) compared with controls. Post hoc analysis revealed that the number of lifetime marijuana use episodes was associated with poorer cognitive function, even after controlling for lifetime alcohol use. The general pattern of results suggested that, even after a month of monitored abstinence, adolescent marijuana users demonstrate subtle neuropsychological deficits compared with nonusers. It is possible that frequent marijuana use during adolescence may negatively influence neuromaturation and cognitive development. (JINS, 2007, 13, 807–820.)

MnFe/NiFe exchange structures have been prepared in an ultra-high vacuum sputtering/surface analysis system. Controlled introduction of residual gas impurities such as O2 and H2O at the MnFe/NiFe interface is studied by in-situ x-ray photoelectron spectroscopy (XPS) and the exchange structures are magnetically characterized. Due to the extreme reactivity of the NiFe surface towards O2, the exchange coupling is severely degraded by only small exposures of this molecule to the NiFe surface. In contrast, H2O does not oxidize the NiFe surface and therefore can be tolerated in greater quantities in the sputtering chamber without detrimental loss of exchange. This understanding of the basic surface chemistry of the MnFe and NiFe surfaces can lead to improved sputtering practices in actual manufacturing applications.

A critical step for successful growth of ultra-high vacuum chemical vapor deposition (UHV-CVD) epitaxial silicon films at low temperatures is the preparation and maintenance of a hydrogen passivated surface. A final 10:1 HF dip has been shown to provide a robust and extremely clean substrate for this purpose; interfacial oxygen and carbon levels of less than 1 × 1013 atoms/cm2 and low levels of structural defects are routinely observed after epitaxial growth. In this work, our understanding of the Si (100) hydrogen passivated surface is reviewed with emphasis on its stability to air and moisture exposure. Additionally, in an effort to reduce interfacial contamination levels even further and explore clusterable processes, other precleaning chemistries such as high pH NH4F and vapor HF have also been evaluated. While surface analysis measurements suggest that the NH4F preparation should be as good or better than the HF dip, the epitaxial film defect density is higher by several orders of magnitude than for the dilute HF preparation. The vapor HF process, while potentially clusterable and therefore desireable, has generally not provided hydrogen passivated surfaces that are clean enough for this application.

We have evaluated the use of HIF vapor cleaning using an Advantage Edge 2000 commercial etcher as a preclean for low temperature UHV/CVD silicon epitaxy (LTE). Surface analysis by contact angle, XPS and Optically Stimulated Electron Emission (OSEE) measurements indicate that vapor HF treated surfaces have higher levels of impurities and are less stable than surfaces dipped in 10:1 diluted HF. Interfacial SIMS measurements and defect analysis using plan-view TEM after epitaxial growth indicate that the vapor treated samples also had higher levels of interfacial oxygen and surface defects. The results indicate that vapor HF precleaning is less effective for LTE than an aqueous HF dip since it provides poorer quality hydrogen passivation of the silicon surface.
